By returning this completed form: - I agree to my son/daughter/child in my care, taking part in the activities of The Cricket School Limited. - I confirm that my child will comply with the ECB Players' Code of Conduct. - I confirm that I will comply with the Parents' & Guardians' Code of Conduct. - I also give consent to the use of photography in the coaching of cricket in respect of my child. - I understand that in the event of any injury or illness, all reasonable steps will be taken to contact me and to deal with that injury/illness appropriately, and it is my responsibility to inform the coach of any known illness/allergy the child may have. - I acknowledge that my safety is my own responsibility when attending indoor and outdoor courses. - PLEASE SEND YOUR CHILD WITH A PACKED LUNCH AND DRINK (AND SUNSCREEN IF APPLICABLE)
